Field RN Case Manager Compensation
Pay rate: $56.14 per hour
Mileage reimbursement: $0.43/mile
Benefits available to full-time employees after 90 days (health, dental, vision, life, short-term disability)
401(k) with company match available after 1 year of service
Field RN Case Manager Highlights
This is a contract assignment with potential to hire permanently based on attendance, performance, and business needs
Required availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
Rotational weekend call : once per month
Counties covered: Independence, Izard, Sharp, Jackson, White, Cleburne, Lawrence, Randolph, Fulton, Woodruff, Craighead
Responsibilities
Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to implement individualized care plans
Identify and reassess appropriate and cost-effective levels of care
Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals
Arrange non-medical support services (housing, transportation) to ensure treatment compliance
Engage specialty resources to resolve complex care needs
Maintain detailed records of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Prevent adverse patient events and intervene promptly when necessary
Advocate for patients while maintaining privacy and confidentiality
Requirements
Education : Associate’s or Bachelor’s Degree in Nursing (or related field)
Licenses : Active, unrestricted Registered Nurse license (Arkansas RN or compact license accepted)
Attendance : Mandatory attendance for the first 90 days
Strong clinical assessment and care coordination skills
Ability to analyze complex medical information and make sound clinical decisions
Case Management certification preferred
