Job Title: Field Care Manager - Nurse 2

Location: Macomb Township, MI 48044 (Hybrid – Michigan (75% Travel to Wayne or Macomb County)

Duration: 3+ Months+ contract (Opportunity for Extension/Permanent based on performance & business needs)

Schedule Notes: Monday – Friday | (40 hours/week)

Job Description:

· Manages a caseload of Medicare/Medicaid members and completes in-home and telephonic assessments.

· Develops, implements, and modifies Individual Care Plans in collaboration with the Interdisciplinary Care Team (ICT).

· Provides clinical support for medically complex members and ensures services are delivered in the least restrictive setting.

· Coordinates community resources including social, housing, and healthcare services to support optimal member outcomes.

· Reviews and completes medical complexity attestations and clinical oversight documentation.

· Serves as primary point of contact for members, caregivers, PCPs, and care team participants.

Required Skills:

· Minimum 2 years of RN experience in healthcare and/or case management.

· Experience with adult population and disease management.

· Knowledge of community health and social service resources.

· Strong assessment, care coordination, and critical thinking skills.

· Ability to work independently with minimal supervision.

· Proficient in EMR systems, Microsoft Word, Outlook, and Excel.

· Must be able to travel 75% for face-to-face home visits.

Required Certifications & Licensure:

·         Active Michigan RN License.

·         Valid Driver’s License, reliable transportation & required auto insurance (100/300/100 limits).

Education:

·         Graduate of an accredited School of Nursing required.

Schedule Note:

·         Monday – Friday

·         Must reside in Michigan

·         Home office with high-speed internet required

·         TB screening required
