Case Management Coordinator Compensation
- Pay rate: $34.13/hour
Case Management Coordinator Highlights
- Location: Remote – Candidates must reside in North Miami, Aventura, Hialeah, or Miramar, FL
- Schedule: Monday – Friday | 8:00 AM – 5:00 PM
- Training: Conducted remotely via Microsoft Teams
- Travel: 50–75% travel within Miami-Dade County for face-to-face member visits
- Work Setting: Remote with field visits to member homes, assisted living facilities, and nursing homes
- Mileage reimbursement provided
Case Management Coordinator Responsibilities
- Conduct telephonic and face-to-face case management assessments with members
- Evaluate member medical needs and develop care coordination plans
- Manage a caseload that may include supportive and medically complex members
- Coordinate services with primary care providers and other healthcare professionals
- Assist with prior authorizations and medication reviews
- Connect members with community resources and support services
- Educate and empower members to improve compliance and promote patient advocacy
- Identify quality and compliance improvement opportunities
- Support optimal health outcomes through proactive care coordination
- Utilize Microsoft Office tools, including Excel, for documentation and reporting
Case Management Coordinator Requirements
- Fluent bilingual communication in English and Spanish (spoken and written)
- Bachelor’s degree required (nursing or social work degree not required)
- Case management experience required
- High School Diploma or GED required
- Proficiency with Microsoft Office, including Excel
- Ability to travel 50–75% within the Miami-Dade region for member visits
- Must reside in North Miami, Aventura, Hialeah, or Miramar, FL
Preferred Qualifications
- Experience working with long-term care populations
- Background in care coordination or managed care
