Are you a dedicated Nurse Practitioner passionate about providing high-quality healthcare to diverse patient populations? We are seeking an experienced Family Practice Nurse Practitioner to join our dynamic team. This role offers the opportunity to work in a flexible, assessment-only capacity—perfect for clinicians who thrive in telehealth, home health, or a hybrid setting. With a focus on patient risk assessments and chronic condition reviews, you'll play a vital role in delivering compassionate, patient-centered care within an innovative and supportive environment.
Required Skills:
- Valid and active Kentucky Medical License
- Enrolled and credentialed in Medicard
- Excellent communication and interpersonal skills
- Ability to perform comprehensive risk assessments and chronic condition reviews
- Comfortable seeing both pediatric and adult patients
- Reliable and comfortable with telehealth and home visits
- Strong organizational skills and self-motivation to manage scheduled visits efficiently
Nice to Have Skills:
- Previous experience with telehealth platforms
- Knowledge of home health protocols
- Bilingual capabilities
- Familiarity with Salesforce or similar scheduling systems
Preferred Education and Experience:
- Master’s degree or higher in Nursing or related healthcare field
- Minimum of 2 years of nursing experience, preferably in family practice, primary care, or telehealth settings
- Current Certification as a Family Nurse Practitioner (FNP)
Other Requirements:
- Availability for full-time, day shift work (Monday to Friday, 8 am – 5 pm)
- Equipment will be mailed and provided by the client
- Ability to perform 12-14 telehealth visits or 6-7 home visits per day, or a combination thereof
- Comfortable working within a schedule released 2-3 weeks in advance
- Willingness to complete ongoing education modules during downtime
- Must meet all licensure, certification, and travel reimbursement requirements as applicable
Compensation and Benefits:
Hourly Pay Rate: $95.48
