Job Details
We’re looking for a Family Nurse Practitioner who is energized by meaningful work and thrives in a team-based care environment. In this role, you’ll provide comprehensive primary care services including patient evaluations, health histories, physical exams, preventive screenings, and treatment for a wide range of acute and chronic conditions.
You’ll collaborate closely with physicians and interdisciplinary care teams to coordinate patient care, provide health education, and support patients navigating complex health challenges. The role also includes addressing behavioral health and substance use concerns, facilitating referrals when appropriate, and ensuring continuity of care for individuals with ongoing medical needs.
This position offers the opportunity to practice medicine in a community-focused setting where patients are supported holistically.
