This is a full-time outpatient Family Nurse Practitioner role providing primary care to a diverse, underserved community. You’ll work in a collaborative, team-based environment with strong clinical support, a predictable weekday schedule, and the opportunity to build long-term patient relationships — all while making a real impact on community health.

Compensation
Pay Range:$140,000-$160,000 Per Year (pro-rated for part time)
Summary:
• This full-time position involves providing comprehensive primary care in Brooklyn, NY.
• The role requires diagnosing and treating various conditions, managing chronic diseases, and collaborating with healthcare teams.
• Candidates must be familiar with electronic medical records and have the relevant qualifications to practice as a Family Nurse Practitioner or Physician Assistant.
Requirements
• Graduate of an accredited PA program
• Active PA license in New York
• NCCPA certification
• DEA registration or ability to obtain
• BLS/ACLS certification (BLS minimum)
• Eligible for credentialing with Medicaid, Medicare, and managed care plans
Qualifications
• Primary care experience in family medicine, internal medicine, or community health
• Experience with underserved or safety-net populations
• Comfort with high patient volume
• Experience with chronic disease management
• Spanish or other second language (huge plus)
• Experience in team-based or integrated care models
• Familiarity with value-based care and quality metrics
