Family Nurse Practitioner {167462}

Position Summary
The Nurse Practitioner will collaborate with a dedicated clinical care team to deliver evidence-based primary and family care to a defined panel of patients. The care team partners daily with providers to drive improved health outcomes through comprehensive assessment, diagnosis, treatment, chronic disease management, health counseling, and disposition planning.

Schedule:

• 32.5 hours per week (average)
• Includes every other weekend
• Possible holiday rotations

Pay: $74.87/hr paid weekly

Key Responsibilities

• Provide evidence-based primary and family care, including:
• • Accurate assessment, diagnosis, and treatment
  • Management of acute and chronic conditions
  • Health counseling and patient education
  • Disposition planning
• Perform routine examinations and diagnostic testing
• Coordinate with healthcare professionals to ensure comprehensive care plans
• Encourage patients to actively participate in their healthcare through screenings, wellness services, and chronic condition management
• Conduct administrative duties including:
• • Opening and closing the clinic
  • Inventory management
  • Insurance verification and payment collection
  • Patient follow-up
  • Maintaining a clean and safe clinical environment

Required Qualifications

• Master’s Degree from an accredited Family Nurse Practitioner program
• Current National Board Certification
• Active, unrestricted Advanced Practice Nurse license in the state of employment
• Active and unrestricted NP/PA license in good standing
• Current BLS certification from ARC or AHA (must provide copy upon offer acceptance)
• DEA registration application required upon offer acceptance (unless otherwise noted)
• Verifiable High School Diploma or GED

Preferred Qualifications

• Minimum of 1 year recent primary care experience as a Nurse Practitioner in family medicine or primary care practice
• • If no primary care experience, successful completion of primary care training program required
• Working knowledge of quality measures in chronic disease management
• Experience with Electronic Medical Systems (EMS)
• Ability to practice autonomously without direct supervision
• Strong organizational and multi-tasking skills
• Excellent verbal, written, and electronic communication skills
• Self-motivated with strong problem-solving ability
• Comfortable working remotely within a clinical care team culture
• Proficient in information management and technology
• Willingness to obtain multi-state licensure
• Ability to collaborate frequently with professional colleagues to ensure quality patient care
