A EHE Medical Care Manager contributes to AHF by utilizing their experience, skills, and education together with a thorough understanding of the “Ending the HIV Epidemic” program, along with knowledge of Ryan White within their specific area. With a caring and knowledgeable focus, you will work with the EHE RN Medical Care Manager, the medical providers, and the Healthcare Center team so that every EHE client receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly within the Healthcare Center, you will meet with clients and complete Needs Assessments, develop Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications. You will reengage clients who dropped out of care or provide support to new patients determined to be at high risk of dropping out based on risk factor assessments, missed appointment data, and service utilization data. For EHE contracts that combine psychosocial needs within EHE Medical Care Management, you will understand mental health needs and basic needs such as housing, food, and clothing, and develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for your patients who lack those services. You will also conduct ongoing retention calls to ensure no gap in retention efforts and to uphold radical cooperation across divisions.

 

An EHE Medical Care Manager enhances the reputation of AHF by utilizing your case management experience. You will meet our clients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your skills of collaboration and engagement, you will coordinate care for people living with HIV (PLWH) eligibility and ensure they achieve and maintain viral load (VL) suppression and retain medical care. You will also bring the essence of the Ending the HIV Epidemic Program and the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. By establishing Needs Assessments, Action/Care Plans, referrals, and support, your work will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services. You will assure the coordination, follow-up, and completion of medical treatments and/or care plan interventions so that these services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and other key family members’ needs and personal support systems. You will verify the clients eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

 

The EHE Medical Care Manager will work as a part of a close-knit team to provide brief interventions focused on a long-term achievable goal. The client and the EHE MCM work together each step of the way towards viral suppression, better health outcomes and higher quality of life. You will collaborate with providers to mutually develop and establish an action or individual service plan with each client that identifies patient needs, goals, and planned courses of action to promote readiness for and adherence to complete HIV/AIDS treatments (i.e., counseling, special programs), and meets identified needs; monitors and documents the changing needs of the client through personal contact and by reviewing the care plan at least every six months.

 

Licenses and Certifications: Bachelor or Master’s degree in Social Science, Social Work, or Human Services. A minimum of 3 years of Case Management experience is preferred.

Knowledge of Ryan White Services is preferable. Knowledge of community resources
