The Director of Case Management provides strategic leadership and operational oversight for the Case Management department, aligning initiatives to deliver consistent clinical, service, and financial outcomes through exemplar practices.
Qualifications:
- Registered Nurse (RN) required
- Bachelor degree required
- BSN and/or Master’s degree (MSN, MBA, MHA) preferred
- CCM certification preferred
- Progressive Director of Case Management experience in an Acute Care Hospital setting required
- Large Hospital Facility oversight strongly preferred
Responsibilities:
- Provide direction and leadership to Case Management managers and staff, ensuring quality, functional excellence, and achievement of strategic/operational objectives
- Drive consistent clinical, service, and financial outcomes through implementation of best-practice standards
- Oversee Case Management, Utilization Management, Denial Management, throughput, Discharge Planning, and LOS reduction initiatives
- Ensure regulatory compliance (CMS, Joint Commission, IMM/MOON) and survey readiness
- Collaborate with physicians, nursing, and multidisciplinary teams to optimize care delivery and patient progression
- Manage budget, Resource Utilization, and Case Management, interdisciplinary cooperation, and data-driven decision-making activities
