Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy for orthopedic, neurologic, and mobility conditions
- Monitor patient progress and adjust care plans as needed
- Collaborate with interdisciplinary care teams
- Maintain accurate and timely clinical documentation
- Participate in discharge planning and continuity of care
Qualifications
- Graduate of an accredited Physical Therapy program
- Active, unrestricted Florida Physical Therapist license
- Skilled nursing or rehabilitation experience preferred
- Strong communication and organizational skills
