Coordinator -ACT Team

(Assertive Community Treatment)

Full Time/ 40 hours

Monday - Thursday: 7:30am - 5:30pm and Friday 8:00am-12:00pm

Competencies:

  • Proven ability to work as a team member and leader with physicians, therapists, and case managers to coordinate care necessary to manage psychiatric and medical issues of ACT consumers. Ability to coordinate a program that emphasizes both aspects as equally important in the recovery of the consumer.
  • Proven ability to work with other healthcare or community agencies to coordinate all aspects of the consumer's continuum of care, focusing on medical as well as psychiatric needs
  • Proven ability to educate staff and consumers about medications and medical conditions that often present themselves in intensive community psychiatric settings
  • Proven ability to assess consumers and distinguish whether presenting symptoms are psychiatric or possibly a psychiatric presentation of medical concerns that require intervention
  • In-depth knowledge of medications: medical - purpose for usage, administration details, possible complications, side effects, interactions; psychotropic - purpose for usage, administration details, possible complications, side effects, interactions
  • In-depth knowledge of physiological responses to medical illnesses and treatment and psychiatric illnesses and treatment
  • Proven superior psychotherapeutic skills
  • Excellent ability to work with others in a cooperative, supportive manner
  • Must possess effective understanding of training techniques used with the SMI population
  • Must possess excellent verbal and written communication skills
  • Must be able to organize and prioritizea variety of tasks
  • Must possess excellent ability to comprehend and maintain program standards
  • Must be able to effectively manage, organize and schedule staff to ensure a successful program and services to ACT consumers

Essential Functions/Responsibilities:

  • Monitor documentation for the ACT Program to ensure admission assessments, treatment plans/reviews, group therapy/individual therapy notes, discharge summaries, nursing assessments, daily cliical staff notes, MD assessments and notes, and medication administration are appropriate and complete
  • Ensure accurate and up-to-date files on al consumers and completion of documentation in a timely manner.
  • Meet or exceed productivity requirements of 110 billable hours per month.
  • Supervise staff as the identify medical and psychiatric concerns and development of therapeutic plans to treat and monitor
  • Provide collaboration and consultation to community agencies as needed
  • Work with other agency staff to maintain movement throughthe program, recognizing progressive movement toward independence
  • Take an active lead in the development of necessary forms and policies for each program, coordinating with other Indian Rivers departments
  • Transport consumers in a company vehicle, ensuring that company vehicle is operated in a safe manner and that appropriate staff are notified if there is an accident or if the vehicle is in need of service
  • If a company vehicle is not available utilize personal vehicle to accomplish work related duties. Indian Rivers will reimburse mileage.
  • Provides outreach community services (may require home visits) as required by supervisor or program standards
  • Maintain courteous and respectful attitudes towards consumers, visitors, and co-workers
  • Work with Human Resources on al staffing and employee disciplinary issues. Monitor absenteeism incidences or performance problems and implement disciplinary actions in collaboration with HR.
  • SupportOrganizational Code of Conduct, Standards Compliance, HIPAA, and Security Programs.
  • Maintain the confidential nature of all consumer and Organization related activities by not divulging information outside the facility.
  • Adhere to attendance and punctuality policy by allowing adequate time to arrive, clock-in, and assume responsibilities at designated time.
  • Comply with all standards related to safety and health.
  • Identify opportunities for improving the quality of services provided by the department and the organization.
  • All other duties and responsibilities as assigned by the supervisor or Executive Director.

Minimum Qualifications:

  • Master's degree in behavioralhealth (licensure preferred)with two (2) yearsexperience inmental health related field required
  • Two (2) year supervisory experience preferred
  • ValidAlabama Driver's License and withgood driving record as defined by Indian Rivers policy
  • Must be able to read, write legibly, speak, and comprehend English.
