Community Health Worker CB#

Summary of Responsibilities:

Jessie Trice Community Health System is seeking a Community Health Worker that will work with the interdisciplinary Medical Home Team, to support the medical provider in ensuring follow-up of patients with chronic conditions that include hypertension and diabetes. Assist patients by organizing conducting and coordinating patient flow, through prioritizing patient queue as referred into the chronic disease remote monitoring, in-reach and outreach programs, maintains effective communication with patients, assist with conducting small groups, provides patient education, and attends community outreach events with focus on hypertension and diabetes. Emphasis will also be placed on food security and other social determinants of health.

Details of Education and Skills:  

 High School Diploma or GED and /or  combination of 3 years health/social services experience and/or education may be substituted ( Some college or Associates preferred).

Excellent people and communication skills ( oral & written)

Must be computer proficient to be able to link patients to resources and utilize EHR system.

Knowledge of local population demographics, assets, needs, community health disparities such as diabetes and hypertension and the ability to connect with and provide ongoing support in a behavior change process.

Strong organizational and time management skills. 

Special Requirement:

Community Health Worker certification ( is a plus)

 Bilingual (English/ Creole or English/ Spanish)
