Penn Medicine is seekig a full-time, day shift Clinical Pharmacogenomics Pharmacist to support the Hospital of the University of Pennsylvania (HUP).
The clinical pharmacogenomics (PGx) pharmacist will supervise the delivery of safe and effective medications by utilizing PGx information in collaboration with the patient's care team. The pharmacist will support and grow the multi-disciplinary PGx clinic within the Division of Translational Medicine and Human Genetics (TMHG) and the PGx consult service. Activities include evaluating, monitoring, and modifying drug therapy; providing recommendations to optimize patient outcomes; educating patients on PGx; minimizing safety issues; ensuring cost-effectiveness; and providing drug information to medical personnel. They will lead and develop implementation plans to integrate PGx testing into clinical workflows and support research initiatives within the Penn Center for Genomic Medicine. They will work closely with the Information Services department to create tools in Epic to facilitate the application of PGx. They will serve as the liaison with the Penn Medicine pharmacy department. The pharmacist should be an effective communicator and be able to integrate themselves within the health team and current structure of the Division of TMHG. Additionally, the candidate should coordinate pharmacotherapy activities and educational opportunities for health system physicians, pharmacists, and residents related to PGx. Finally, the clinical pharmacy specialist should fulfill requirements associated with hospital and department policies and procedures, JCAHO, and other regulatory agencies.
Credentials:
- PA Pharmacist Licensure (Required)
- Board of Pharmaceutical Specialties Certification in area of specialty practice (or Pharmacotherapy if no specialty certification available) Must be obtained within 1 year of hire
Education or Equivalent Experience:
- Bachelor of Science Degree or Doctor of Pharmacy Degree from an accredited College of Pharmacy (Required)
- Accredited PGY1 Pharmacy Residency or 10 years as a licensed pharmacist with 50% of time spent in this specialty (Required)
- Accredited PGY2 Pharmacy Residency or 3+ years as a licensed pharmacist with 50% of time spent in this specialty (Preferred)
