Summary
We are seeking a highly skilled Clinical Pharmacist with expertise in Total Parenteral Nutrition (TPN) to join our clients specialty pharmacy team. The ideal candidate will provide exceptional patient care, collaborate with healthcare professionals, and ensure compliance with home infusion pharmacy protocols. This role involves direct patient assessment, prescription verification, medication management, and leadership in driving clinical initiatives and operational excellence.
Key Responsibilities
Conduct thorough patient assessments to determine suitability for home infusion therapy, including reviewing medical records and lab results.Collaborate with nursing staff to ensure home health nurse involvement when necessary before initiating therapy.
Work closely with physicians, nurses, discharge planners, and other healthcare professionals to coordinate seamless patient transitions to home infusion therapy.
Verify insurance coverage, obtain necessary authorizations, and explore alternative options if required.
Review prescriptions for accuracy, completeness, and appropriateness, ensuring compliance with legal and regulatory requirements.
Communicate with healthcare providers to clarify prescription details or obtain additional information as needed.
Draft and customizeTPN formulations based on clinical guidelines and patient-specific needs.
Maintain accurate and up-to-date patient profiles, including medical history, allergies, and other pertinent information.
Oversee inventory management, ensuring an adequate supply of infusion medications and related materials.
Ensure sterile compounding and dispensing of medications in accordance with guidelines and physician orders.
Monitor medication therapyoutcomes, identify potential issues, and make necessary adjustments.
Document medication errors for Quality Assurance review and ensure compliance with safety protocols.
Participate in surveys and inspections conducted by authorized regulatory agencies.
Manage pharmaceutical recalls and report product incidents as required.
Engage in performance improvement initiatives and continuing education to stay current with clinical advancements.
Assume on-call responsibilities when required to support patient care needs.
Qualifications & Requirements
Doctor of Pharmacy (PharmD) degree from an accredited institution.
Active pharmacist license in California
Specialty pharmacy experience, with a focus on TPN compounding.
Strong knowledge of clinical pharmacokinetics, fluid/electrolyte management, and home infusion therapy.
Ability to manage multiple complex projects simultaneously while ensuring high accuracy.
Exceptional organizational, problem-solving, and communication skills.
Proficiency in Microsoft Office and pharmacy management systems.
