Clinical Pharmacist – MTM (Medication Therapy Management) REMOTE {168246}

Position Overview:
We are seeking an experienced Clinical Pharmacist to join our Medication Therapy Management (MTM) team. In this role, you will conduct comprehensive medication reviews (CMRs), perform outbound patient outreach, and provide clinical consultations focused on statin therapy aligned with AHA guidelines.

This position plays a critical role in improving patient outcomes by ensuring accurate documentation, supporting Part D Star measures, and delivering high-quality MTM services. The pharmacist will also transcribe patient-specific clinical data into a third-party web-based system to generate standardized patient reports.

Key Responsibilities:

• Conduct high-volume outbound calls (approximately 100 calls per day) to patients
• Perform comprehensive medication reviews (CMRs)
• Deliver medication therapy management (MTM) services
• Provide clinical guidance on statin therapy in alignment with AHA guidelines
• Accurately document prescription, diagnosis, and recommendation data in a web-based platform
• Transcribe information from hard-copy or internal applications into standardized documentation
• Support initiatives related to Medicare Part D Star measures (SUPD, SPC, CMR)
• Maintain patient confidentiality and adhere to all compliance standards

Qualifications:

Education & Licensing:

• Active Pharmacist License in the state of Florida 

Experience:

• Minimum of 4 years of experience as a clinical MTM pharmacist
• Management experience preferred
• Experience with Medicare Part D Star measures (SUPD, SPC, CMR)
• Strong understanding of pharmaceutical and medical terminology
• Clinical experience in medication review required
