Job Summary and Qualifications
The Internal Medicine Clinical Pharmacist is responsible for the provision of comprehensive clinical pharmacy services for the including pharmacokinetic and therapeutic consultation with the primary objective to assure safe and effective use of medications.
What you will do in this role:
- Provide pharmacy review of medication orders prior to administration.
- Work collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services.
- Participate in the development of medication use management programs within HCA Florida- Brandon Hospital including clinical guidelines, critical pathways, disease management, and drug use programs.
- Participates in providing didactic and experiential training to pharmacy residents, GME residents and pharmacy students.
- For patients involving High-Risk Medications and procedures, will participate in medication reconciliation and in selecting and recommending the right dose monitoring therapy to ensure patient safety.
- Participate in the professional development and competency of clinical staff. Functions as a role model and as an educational resource for pharmacy staff.
- Assist in the development, implementation, and evaluation of residency programs in the areas of Pharmaceutical Care and Clinical Pharmacology.
- Assure compliance with all local, state and federal laws covering the storage, handling and dispensing of drugs and for interpreting each prescription order to determine that it meets all legal requirements.
- Participates collegially in the development of new programs, services and practices, in the educational activities of the department and the management and administration of the department.
- Provides patient specific medication use teaching for high risk patients discharged when appropriate.
- Focus on cost avoidance, formulary management and cost savings for medications use in the facility.
- Participate in decision making committee, workgroups impacting policies and procedures affecting adult, inpatient setting.
- Actively participates in clinical research projects resulting in presentations and/or publications.
- Performs other related duties as required.
What qualifications you will need:
- Must be licensed or eligible for licensure in Florida State and meet the following criteria:
- Doctorate in Pharmacy (PharmD) or Bachelors of Science in Pharmacy
- 3 years of experience in area of specialty or completion of PGY-1 residency with 1 year of experience of or completion of PGY-2 pharmacy residency or fellowship
- Board Certified Pharmacotherapy Specialist (BCPS) or other nationally accredited certification required to obtain within a year of hire
