Summary:
To provide pharmaceutical care in order to assure the safe, effective, and cost-effective use of drugs for Penn Medicine patients in an outpatient setting.
Responsibilities:
•
Timely completion of all work assignments.
•
Autonomous work organization and completion.
•
Communicates appropriately with immediate coworkers and all other health care staff: Pharmacists, Nurses, Patients and Students, and listens carefully to others in order to respond appropriately.
•
Calculates correctly and accurately amount of medication to be dispensed based on days supply allowed, age, etc.
•
Monitors patient profiles, drug utilization, and insurance carrier messages to assure medication compliance, patient specific parameters are considered in medication preparation.
•
Participates in Entity and Department wide initiatives for Patient /Employee safety.
•
Demonstrates an awareness of patient/ employee safety when carrying out daily responsibilities of their position.
•
Validation of annual competencies required for the position as assigned.
•
Complies in all activities with accreditation, legal, regulatory, and safety requirements for specific practice setting(s).
•
Complies with all third-party insurance plan rules and regulations.
•
Assumes responsibility for compliance with all health system policies and procedures.
•
Participates in the established process for assessing, managing, and reporting ADEs.
•
Demonstrates proficiency on all pharmacy systems applicable to individual role.
•
Provides support for other staff members toward their overall effectiveness.
•
Trains new hire pharmacists, interns, and technicians.
•
Contributes to the achievement of departmental goals.
•
Takes responsibility for their own professional development.
•
Exhibit professionalism at all times to include a clean professional image, punctual arrival to work and return from breaks.
•
Operational Staff:
- Escalates to embedded office team members when necessary
- Prepares and dispenses medications using appropriate techniques to ensure accuracy, optimal outcomes of drug therapies
- Keeps active, accurate inventory in work area and appropriately restocks area with returned medications per policy to assure workstations are organized at all times

Clinical Staff:
- Designs, recommends, monitors, evaluates and documents patient and age-specific pharmacotherapy
Credentials:
- Pharmacist (Required)
Education or Equivalent Experience:
- Ph.D. (Required)
- harmD or RPH in Pharmacy PGY1 Residency Training in Pharmacy
- Other
- PGY2 Residency Training in Ambulatory Care / Pharmacotherapy, etc preferred PA Authorization to Administer Injectables License preferred NJ Authorized Immunization License preferred
