Client Care Assistant

Summary:

The Client Care Assistant works closely with each of the Clients and the outside Medical providers, onan individual basis providing assistance in the areas of, medication observation and medical appointmentscheduling, as well as overall client health. The Client Care Assistant ensures that all informationobtained during the course of the client's stay is organized and facilitated appropriately. The Client CareAssistant adheres to all state and federal regulations and accreditation agency standards applicable tothe individual licensure and narcotic treatment programs.

Job Duties:

  • Observation, documentation, audit functions, and record keeping of all

observed/inventoried/destroyed/prescribed medications

  • Assessment of clients with undiagnosed or diagnosed conditions, and assisting in ascertainingclinical medication management with the Client Care Director and Medical Director.
  • Observation, documentation, and record keeping of client health status.
  • Works closely with Operations Manager/Medical Director/Therapist/ and Case Managers onclient health status and presenting conditions.
  • Manages Client Care data in the HER such as orders,
  • Pick up medications/ other medical supplies/ interact with client's Pharmacy/Pharmacist
  • Set all medical appointments for clients including medical, dental, and psychiatric as needed.
  • Work with Operations on Medication refills
  • Communicate with local hospitals, doctor's offices, urgent cares, etc.
  • Communicate with families when release is valid for updates as needed.
  • Responsible for all medical records requests from outside providers while in our care.
  • Understanding and ability to score COWS and CIWA for withdrawal of symptoms during detox.Utilize the 6 Rights when before clients self-administering medication.
  • Ensure that an adequate supply of medications, supplies, and equipment is on hand to meet theneeds of the clients.
  • Ensure that narcotic records are accurate for your shift. Narcotic count must be done between allshifts and after each self-administration of said medication.
  • Dispose of drugs and narcotics as required, and in accordance with federal and state guidelinesand company established procedures.
  • Make daily rounds of your unit/shift to ensure clients are doing well.
  • Act as a liaison with MD/NP/PA.
  • Be available for staff regarding questions or issues.

Completion of all client process duties and/or documentation within timelines:

  • Responds to all social service needs, requests by physicians, legal issues, and family needs.Required documentation in chart according to policy timelines.
  • Skills will be measured by management through regular routine quantitative and qualitativechart audits. Chart audits will reflect appropriate content and timeline benchmarks of

completion.

  • Updates to family and/or referral sources (as allowed via client consents).
  • Crisis Intervention
  • Observe, evaluate and report on behavior, attitudes, general emotional stability, clients and theirfamily members to help assist in treatment planning/discharge planning by assessing socialneeds of individual clients including referrals to outside providers after transition to outpatienttherapy.
  • Will be provided performance evaluations to measure performance areas. Inability to meetperformance expectations will result in specific, short term performance enhancement actionplans with a timeline to improve. Below average rating in employee performance will result inprogressive disciplinary action up to and including termination of employment.

Minimum Qualifications:

  • Must possess, as a minimum, a RADT or similar credential.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant.
  • Must be knowledgeable of medical practices and procedures, as well as laws, regulations, andguidelines that pertain to residential treatment care.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously withand supervise other personnel.
  • Must be able to communicate/relate information concerning the client's status with familymembers.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must have treatment experience with strong professional references attesting to high degree ofclinical sophistication.
  • Trained to independently diagnose and treat mental health and substance abuse problemsCurrent certification in First Aid/CPR
  • Advanced knowledge of HIPAA and CFR 42 Requirements

Valid California Driver's License
