LTSS Service Care Manager (RN or LCSW) Compensation

  • The pay for this position is $40.00 - $42.00 per hour
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

LTSS Service Care Manager (RN or LCSW) Highlights

  • The required availability for this position is Monday–Friday, 8:00am–5:00pm EST
  • This is a field-based remote role requiring travel up to 80%
  • Candidates must reside within a 2.5–3 hour radius of Jacksonville, NC
  • Mileage and lodging are reimbursed when required for member visits
  • Potential to extend after initial assignment

LTSS Service Care Manager (RN or LCSW) Responsibilities

  • Manage a caseload of healthcare members with long-term support and service needs
  • Conduct in-person and telephonic member assessments
  • Develop, implement, and monitor individualized care plans
  • Coordinate referrals, discharge planning, and integration of services
  • Ensure timely and appropriate delivery of services and close gaps in care
  • Assist members with appeals and complaint resolution
  • Collaborate with providers, discharge planners, and family members

LTSS Service Care Manager (RN or LCSW) Requirements

  • Active RN license or LCSW in North Carolina
  • Valid driver’s license
  • Minimum 2 years of care management experience
  • Strong background in physical health, utilization management, or home health
  • Proficiency in Microsoft Office and experience using electronic health records

LTSS Service Care Manager (RN or LCSW) Preferences

  • 4–6+ years of care management experience
  • Experience in long-term care, home health, or discharge planning
