Position Overview
The Care Manager (CM) plays a key role in reducing overall admissions, readmissions, and emergency room visits. This position supports patients with high‑risk health conditions and helps them navigate the healthcare system effectively. The CM promotes patient empowerment by serving as an educator, advocate, and resource for patients and their families, ensuring the highest possible quality of life.
The CM works collaboratively with a multidisciplinary care team—including physicians, nurses, pharmacists, laboratory technologists, social workers, and health educators—to coordinate and support patient care.
Top Responsibilities
- Support initiatives to reduce patient admissions, readmissions, and ER utilization.
- Assist high‑risk patients in navigating the healthcare system and accessing appropriate resources.
- Provide patient education, support, and advocacy to enhance self‑management and quality of life.
- Collaborate with multidisciplinary healthcare teams to coordinate comprehensive patient care.
- Conduct assessments, monitor patient progress, and ensure adherence to care plans.
- Document all patient interactions, care coordination activities, and clinical updates accurately.
- Utilize clinical knowledge to identify patient needs and escalate concerns when necessary.
- Maintain compliance with HIPAA and organizational policies.
Required Skills & Qualifications
- Registered Nurse (RN) license in the state of Florida (required).
- BLS (Basic Life Support) certification (required).
- Minimum of 2–3 years of healthcare experience in a hospital, urgent care, or primary care setting.
- Medicare Advantage care experience strongly preferred.
- Strong verbal communication and customer service skills; ability to interact professionally with patients and staff at all levels.
- Ability to work independently with minimal supervision.
- Ability to work in a fast‑paced environment; strong multi‑tasking, organizational, and time‑management skills.
- Proficiency in Microsoft Office, including Excel.
- Working knowledge of medical terminology.
- Familiarity with medical equipment and willingness to learn basic maintenance procedures.
- Knowledge of HIPAA regulations and patient confidentiality practices.
- Ability to complete a color‑blindness assessment if required.
Preferred (Nice-to-Have) Qualifications
- Bilingual (Spanish/English) – strongly preferred and highly valued.
- IV certification.
- Experience with care management in Medicare Advantage programs.
Work Environment & Additional Details
- Work Days: Monday–Friday (5 days per week).
- Hours: 8:00am–5:00pm.
- Travel Required: No.
- Multiple Locations: No – single-site role.
- In-Home Care: No home visits required.
