Healthcare Case Manager (RN)

Location: Kissimmee, FL (4 Facilities) 

Worksite Type: Fully On‑Site 

Employment Type: Contract (Approx. 4–6 Months) 

Shift: Monday–Friday, 8:00 AM – 5:00 PM 

About the Role

We are seeking an experienced Registered Nurse Case Manager to support an Institutional Special Needs Plan (ISNP) program. This is a fully on‑site position covering four long‑term care facilities within Osceola County, FL. The assignment is to provide temporary coverage for an FMLA leave.

The RN Case Manager will work closely with interdisciplinary care teams to ensure high‑quality care coordination, accurate documentation, and proactive management of members’ health needs.

Primary Responsibilities

• Maintain accurate and complete clinical documentation.

• Collaborate with interdisciplinary care teams and facility staff.

• Support care coordination activities, including Medicare Part A, B, and D benefit determination.

• Identify and respond to changes in patient condition in partnership with the advanced practice clinician.

• Assist with therapy coordination and participate in facility‑based meetings.

• Schedule and participate in family conferences, team meetings, and case presentations.

• Support training, tracking, and compliance with quality measures.

• Conduct or assist with in‑services for facility staff.

• Assist providers with documentation, coding accuracy, medication reconciliation, assessments, and nursing rounds.

• Write verbal orders per state RN practice laws and facility policies.

• Manage members experiencing acute medical issues or exacerbations of chronic conditions.

• Support members during short‑term sub‑acute rehabilitation episodes.

Required Qualifications

• Active, unrestricted RN license in state of residence (or ability to obtain by start date).

• Associate degree or higher in Nursing.

• Current Basic Life Support (BLS) certification.

• Ability to lift a 30‑lb bag and perform physical assessments.

• Reliable transportation.

• Full COVID‑19 vaccination (per regulatory and facility requirements).

• Ability to pass a pre‑employment drug screen.

Preferred Qualifications

• 1+ year of clinical experience (long‑term care preferred).

• Experience supporting patients with complex behavioral, social, or functional needs.

• Knowledge of geriatrics and chronic illness management.

• Proficiency with electronic and written medical documentation.

• Strong communication and time‑management skills.

• Ability to work across multiple care settings.

• Ability to travel between facilities during the workday.
