Case Manager RN 

Work Setting: Visiting between Five Skilled Nursing & Rehabilitation Facilities
Employment Type: Contract with Potential for Permanent Hire

About the Role
We are seeking an experienced and compassionate Case Manager RN to support five nursing and rehabilitation facilities. This role focuses on care coordination, clinical collaboration, and supporting patient outcomes in an understaffed environment. The ideal candidate is a proactive clinician who thrives in interdisciplinary settings and is passionate about serving geriatric and chronically ill populations.

Primary Responsibilities
  • Ensure accurate, complete, and timely clinical documentation
  • Communicate and collaborate with the interdisciplinary care team
  • Support providers with various care coordination activities
  • Assist with benefit determination related to Medicare Part A, B, and D
  • Collaborate with advanced practice clinicians and facility staff regarding patient changes in condition
  • Facilitate therapy coordination and participate in facility‑based meetings
  • Schedule and participate in family conferences, team meetings, and case presentations
  • Coordinate training, tracking, and compliance with quality measures
  • Create and conduct in‑services for staff and other selected audiences
  • Support providers with documentation, coding, medication reconciliation, nursing rounds, assessments, and patient documentation
  • Write verbal orders as permitted by state RN practice laws and facility policies
  • Assist in managing patients experiencing acute medical issues or exacerbations of chronic conditions, including diagnostics and interventions
  • Support members during short‑term sub‑acute rehabilitation episodes
Required Qualifications
  • Active and unrestricted RN license in your state of residence, or ability to obtain by start date
  • Associate degree or higher in Nursing
  • Current Basic Life Support (BLS) certification
  • Ability to meet physical requirements, including lifting up to 30 lbs and performing physical assessments
  • Reliable transportation
