Matrix Care Management provides professional case management services for older adults, individuals with special needs, and those recovering from catastrophic injuries.

Our case managers play a vital role in helping clients and families adjust after unexpected life events. Whether advocating for an aging adult who can no longer advocate for themselves or helping families care for a loved one with special needs, our team ensures clients receive the support, resources, and care they deserve.

We collaborate with trust officers, settlement planners, personal injury attorneys, estate planners, and healthcare professionals to create comprehensive care and life planning strategies for every client.

What You’ll Do

· Coordinate care plans for elderly and special needs clients

· Assist clients and families with Medicaid and long-term care applications

· Communicate and collaborate with medical providers, therapists, and care teams

· Prepare documentation and reports to support legal settlements and budgeting plans

· Advocate for clients and ensure access to appropriate community resources

· Develop and implement client-centered care strategies

· Monitor client progress and adjust care plans as needed

Requirements

What We’re Looking For

Required

Bilingual English / Spanish

Preferred

Degree in Social Work or related field

Matrix supports case managers in obtaining professional certifications after joining the team.

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.

Compensation

· Base salary: $50,000 – $60,000 depending on experience

· Miami Dade and/or Broward Office
