What you’ll do
• Coordinate care for individuals enrolled in the Health Homes program
• Develop and maintain person-centered care plans
• Build consistent relationships with members and service providers
• Connect individuals to medical, behavioral health, and community resources
• Complete timely, accurate documentation
• Work independently with strong team and leadership support
You’ll do well here if you
• Have a bachelor’s degree in human services or related experience
• Are organized, dependable, and communicative
• Appreciate clear expectations and manageable workloads
