CASE MANAGER (REMOTE)
Legacy Navigation | Full-Time | Work From Home
Every day, we guide families through the hardest chapters of their lives. We’re looking for someone who was made for this work.
WHO WE ARE
At Legacy Navigation, we bring clarity, strategy, and unwavering advocacy to seniors and their families navigating long-term care and Medicaid planning. These transitions are complex, emotional, and financially overwhelming and families trust us to walk through every step with them.
We are a small, specialized, mission-driven team. We are growing. And we take both seriously.
Our case managers have helped hundreds of families across the country secure Medicaid coverage they couldn’t have navigated alone and build a future they didn’t think was possible.
WHY THIS ROLE MATTERS
This is not a “check-the-box” case management job.
A win here looks like this: a family that came to you overwhelmed, confused, and scared now has a plan. Medicaid is approved. Their loved one is cared for. And they called to say thank you.
A hard day looks like this: a document deadline, an anxious family call, and a Medicaid follow-up, all before noon. You stay calm. You prioritize. You don’t drop the ball.
If both of those sentences make you lean in rather than step back, keep reading.

WHAT YOU’LL DO
As a Case Manager, you’ll carry a caseload of up to approximately 40 clients and guide each family from first contact through final resolution. This role is both strategic and relational, big-picture thinking with meticulous attention to every detail.
Your responsibilities include:
- Build trusting, lasting relationships with clients, families, and care facilities

WHO YOU ARE
We hire for character first, experience second.
Skills can be trained. The right heart can’t.
You are someone who:
- Stays calm when families are scared and becomes their steady ground
- Catches the details others miss, and takes pride in it
- Translates complex topics into plain language so families feel heard and informed
- Thrives in a remote environment because you own your outcomes you don’t wait to be managed
- Genuinely cares about the people you serve, not just the cases you carry
- Leans into complexity rather than away from it
PREFERRED BACKGROUND
No prior Medicaid expertise is required. If the character fits, we will train the skills. These backgrounds tend to translate well:
- Case management, healthcare, social work, or Medicaid experience
- Experience working with seniors or long-term care systems
- Strong administrative or documentation skills
- Comfort discussing finances, income, assets, and strategies with families
- Proficiency with Microsoft Office, Google Docs, or similar tools
OUR CULTURE
We celebrate the day a family calls to say they finally have a plan. That phone call is why we do this.
Our team shows up with excellence, supports one another without being asked, and treats every family like they’re the only family. We communicate honestly. We invest in the people on this team. Your growth here is not an accident, it’s a plan.
This is not a culture of micromanagement or burnout. It is a culture of trust, high standards, and genuine care, for our clients and for each other.
HOW TO APPLY
We read every application carefully. The strongest candidates don’t just have the right experience, they have the right reason for being here.
