Case Manager (Community Treatment Specialist) - Full Time Position

Location: 2 Old County Road, Barrington, RI
Division: Behavioral Health Services

Are you passionate about making a meaningful difference in people's lives? EBCAP is seeking a full-time Case Manager (Community Treatment Specialist) to join our multi-disciplinary Integrated Health Home Team. This rewarding position offers the opportunity to provide essential support services to clients in our community.

What You'll Do

Key Responsibilities:

  • Perform comprehensive case management and care coordination duties on our Community Support Team
  • Provide direct assistance, advocacy, coordination of care, and individualized support to clients
  • Assist clients in scheduling and attending medical, eye, and dental health care appointments
  • Document all encounters in the electronic medical record within 48 hours
  • Complete required clinical documentation including Treatment Plans and Yearly Assessments to meet licensing and regulatory standards
  • Meet established productivity standards
  • Help clients access state and federal entitlements including SSI, SSDI, food stamps, and Medicaid
  • Assist clients in obtaining and maintaining safe and affordable housing

Schedule

This is a full-time position, 40 hours per week.

What We're Looking For:

Qualifications:

  • Bachelor's Degree in Human Services field
  • Valid driver's license, acceptable driving record, and reliable transportation with insurance (travel required)
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a collaborative team
