Job Title: Case Manager – PHP/IOP/OP Program
Work Environment:

Columbus, OH, Evening shift, Monday- Friday, 4:00 PM – 10:00 PM

Position Summary:
The Case Manager is a vital member of the multidisciplinary team, responsible for delivering high-quality, client-centered case management services within our Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP), and Outpatient Program (OP). This role focuses on providing holistic, community-based care to individuals with co-occurring mental health and substance use disorders, ensuring that each client receives comprehensive support that promotes recovery, wellness, and long-term stability.

Key Responsibilities:
  • Client-Centered Support:
    • Conduct comprehensive needs assessments and develop individualized service plans in collaboration with clients, families, and the treatment team.
    • Advocates for client needs and preferences, ensuring their voice is central in all decision-making processes.
  • Holistic Care Coordination:
    • Link clients to appropriate community resources, including housing, vocational support, healthcare, peer support groups, and social services.
    • Coordinate care with mental health, medical, and substance use providers to ensure seamless integration of services.
  • Treatment Planning & Documentation:
    • Participate in treatment team meetings to review progress, update goals, and modify plans as needed.
    • Maintain accurate, timely, and confidential documentation in accordance with program policies and regulatory standards.
  • Community Integration:
    • Foster relationships with local organizations, agencies, and support networks to expand available resources for clients.
    • Promote community engagement opportunities that support recovery, personal growth, and social connectedness.
  • Crisis Support & Intervention:
    • Provide crisis intervention, de-escalation, and support during periods of acute need.
    • Collaborate with the clinical team to ensure appropriate follow-up care and safety planning.

Qualifications:
  • Knowledge of co-occurring mental health and substance use disorders, recovery-oriented systems of care, and trauma-informed practices.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work collaboratively in a multidisciplinary team and independently in the community.
  • Familiarity with local resources, benefits systems, and community supports.
  • Valid driver’s license, reliable transportation, and willingness to travel within the community.
