Case Manager 164 Part Time - $28,919 - Edison/Old Bridge, NJ

Salary - $28,919 per year

Responsibilities:

  • Provide case management services to individuals and families residing in Permanent Supportive Housing sites including completion of intakes, developing service plans, linkages to other services including, conflict de-escalation, managing relationships with a broad range of community providers, promoting housing retention through teaching life skills, setting up programming for tenants.
  • Utilization of HMIS case management software.

Requirements:

  • Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred.
  • Have a familiarity with housing first, progressive engagement, and reality testing. Experience with conflict resolution and mediation.
  • (2) years relevant work experience and demonstrated ability to provide effective services to clients.
  • Needs to be culturally competent across all populations.
  • Bilingual a plus (Spanish).
  • Ability to work independently.
  • May include additional on-call hours with evening hours, flexibility in creating own work schedule.
  • Valid driver’s license with acceptable driving history.
