Case Management Coordinator Compensation

Competitive pay rate up to $34.13 per hour


Case Management Coordinator Highlights

  • Monday–Friday, 8:00am–5:00pm
  • Remote/work from home position
  • Approximately 75% travel within the region (home visits, assisted living facilities, nursing homes)
  • Training conducted remotely via Microsoft Teams

Case Management Coordinator Responsibilities

  • Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program members
  • Conduct telephonic and face-to-face comprehensive member assessments
  • Develop, implement, and monitor individualized care plans
  • Manage a caseload of supportive and medically complex members
  • Coordinate with PCPs, specialists, and skilled providers
  • Assist with prior authorizations, medication review, and condition management
  • Connect members with community resources and supports
  • Identify and escalate quality-of-care concerns
  • Utilize motivational interviewing and negotiation skills to promote member engagement
  • Ensure documentation complies with regulatory and accreditation standards

Case Management Coordinator Requirements

  • Bachelor’s degree required (Social Work or related field) – No nurses
  • Case management experience required
  • Microsoft Office proficiency, including Excel
  • Verifiable High School Diploma or GED required
  • Ability to multitask and adapt in a fast-paced environment
  • Strong verbal and written communication skills
  • Willing and able to travel approximately 75% locally

Preferred Qualifications

  • Long term care experience
  • Bilingual Spanish/English

  • Coordinate care for members with intensive needs
  • Identify individuals at risk and increase the percentage of membership in case management
  • Prepare various case mix reports and special projects as requested by senior management
  • Provide statistical case reviews and generate utilization reports
  • Ensure that case management program objectives are met by evaluating the effectiveness of alternative care services and that cost
  • Identified in the members care plan
