A-Line Staffing is now hiring a Healthcare Consultant I (Case Management Coordinator) for a major Fortune 6 healthcare organization. This is a full-time, long-term opportunity offering career growth while making a direct impact on members throughout Miami-Dade County.

• Pay Rate: $34.13/hour

• Fully remote work-from-home position

• Approximately 75% regional travel for face-to-face member visits

• Full-time: 40 hours per week

• Monday – Friday | 8:00 AM – 5:00 PM EST

• Training conducted remotely via Microsoft Teams (4–6 weeks)

• Candidates must reside in Miami-Dade County, FL (Doral or Coral Gables)

• Bilingual Spanish/English REQUIRED (read, write, speak fluently)

• Conduct telephonic and face-to-face assessments with Medicaid Long Term Care/Comprehensive Program members

• Develop, implement, and monitor individualized care plans

• Coordinate services including:

  • Prior authorizations

  • PCP and skilled provider communication

  • Medication reviews

  • Condition management education

  • Community resources and support services

• Utilize critical thinking, motivational interviewing, and negotiation skills to drive member engagement and outcomes

• Perform comprehensive data review and documentation using care management tools

• Conduct multidisciplinary reviews to achieve optimal member outcomes

• Identify and escalate quality-of-care concerns through established channels

• Provide education, coaching, and support to empower members in healthcare decision-making

• Travel to member homes, Assisted Living Facilities, and Skilled Nursing Facilities throughout the region

• Bachelor’s degree required (Social Work or related field)

• This is NOT a nursing role — no RNs or LPNs

• Case Management experience required

• Long Term Care experience preferred

• Fully bilingual in English & Spanish (verbal and written)

• Proficiency in Microsoft Office (Excel, Outlook, Teams)

• Strong written and verbal communication skills

• Ability to multitask and adapt in a fast-paced environment

• Highly organized, detail-oriented, and able to work independently

• Verifiable High School Diploma or GED required

• Experience with Medicaid or Long-Term Care populations

• Field-based or home-visit case management experience

• Knowledge of community resources within Miami-Dade County
