Care Manager

Cayuga Counseling Services is seeking a Care Manager to provide Care Management for eligible Medicaid members under the Medicaid 1115 wavier program. 

The Care Manager is responsible for the following:

• Provide Care Management services that address the health-related social needs of eligible members under the Medicaid 1115 waiver which can include housing, nutrition and transportation needs.
• Assist eligible members with securing safe, affordable housing.
• Follow up with members to ensure they receive the services they were referred to.
• Refer eligible members to programs within the Network.
• Maintain strict compliance with HIPPA and all federal, state and agency confidentiality regulations, policies and procedures.
• Document all services and outreach per program policies and procedures.
• Attend various meetings to educate providers about the services available under the wavier.
• Attend all required trainings.
• Performs other duties as needed.

Knowledge, Skills, and Abilities:

• Experience with working with Medicaid or vulnerable populations preferred.
• Strong computer skills, including experience with Microsoft Office and electronic documentation systems.
• Knowledge of social services and community resources.
• Strong documentation and organizational skills
• The ability to work independently.

The Agency offers:
