Title: Care Management Supervisor

Department: Care Management

Classification: Exempt

Reports To: Director of Nursing

Direct Reports: RN Care Managers, Clinical Care Coordinator, Behavioral Health Clinical Care Coordinator, Referral Coordinator

Position Summary

Oversees daily operations of the care management team to ensure high-quality, patient-centered care. This position provides leadership, mentorship, and clinical or operational support to their team, ensuring adherence to organizational policies, regulatory requirements, and evidence-based care management practices. The Care Management Supervisor oversees caseloads, assesses program performance, and works closely with interdisciplinary partners to improve outcomes, address barriers to care, and improve patient experience.

 

Key Responsibilities
  • Ensures patients receive appropriate mental health and specialty healthcare services by overseeing the coordination of referrals, assessments, and follow-ups.
  • Oversees/Monitors the billing and coding requirements for Care Management and Care Coordination services with PHPs, to ensure appropriate reimbursement.
  • Plans, organizes, directs, and leads the development of chronic disease program protocols and standards of care.
  • Establishes objectives and evaluative or operational criteria for units managed.
  • Directs or conducts recruitment, hiring, and training of personnel.
  • Ensures the delivery of high-risk care management, chronic care management, Medicare Care Management, and Medicaid care management.
  • Oversees patient compliance, including developing education, training for optimizing call outcomes and driving Physician ordered Care Plan.
  • Consults with medical, business, and community groups to respond to community needs, enhance public relations, coordinate activities and plans, and promote health programs.

 

Minimum Qualifications
  • Registered Nurse (RN) in the state of North Carolina.
  • Minimum 3-5 years of experience in care/case management, care coordination, or related field.
  • At least 2 years of supervisory or managerial experience in a healthcare setting are strongly preferred.
  • Other coursework in Social Work, Psychology, Public Health, or a related field is desirable

 

Other Skills, Knowledge, and Abilities
  • Strong leadership and management skills.
  • Knowledge of primary care nursing and care management, mental health disorders and treatments, and care coordination models.
  • Excellent communication and interpersonal skills to engage with patients, families, and healthcare professionals.
  • Familiarity with electronic health records (EHR) and case management software.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Understanding of HIPAA regulations and patient confidentiality laws.
  • Must be knowledgeable of the concept of Integrated Health Services with a strong focus on primary care.
  • Proficient with Microsoft Suite (Word, Excel, PowerPoint, etc.).
