Care Concierge to join our team and support our geriatric care manager in delivering exceptional care.
Position Overview:
The Care Concierge will work under the direction of a geriatric care manager to assist with daily tasks, including wellness checks, medication refills, accompanying to medical appointments, transportation, social outings, light housekeeping, and personal care. This role is ideal for someone with a background in healthcare, assisted living or direct care who is passionate about supporting older adults and helping them maintain their independence.
Key Responsibilities:
- Assist with wellness checks, including monitoring clients’ physical and emotional well-being.
- Provide transportation for clients to medical appointments, social events, and errands.
- Coordinate and participate in social outings and activities.
- Perform light housekeeping duties to maintain a safe and comfortable living environment for clients.
- Assist with personal care, including grooming, dressing, and other daily tasks.
- Document care provided and report any changes in client condition to the care manager.
- Work collaboratively with the geriatric care manager to ensure clients’ needs are met.
- Maintain confidentiality and adhere to privacy regulations.
Qualifications:
- Preferably a background in a healthcare related field. Medical Assistant (MA), Certified Nurse Aid (CNA) Licensed Practical Nurse (LPN), or similar healthcare role.
- Previous experience working with older adults or in a caregiving role preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Ability to work remotely.
- Proficient in Microsoft office and EMR type software's.
- Compassionate, patient, and attentive to the needs of others.
- Valid driver’s license and reliable transportation required.
- Must be able to pass a background check and drug screening.
