RN Care Manager Details:

• Location: Combination of remote work and field work within County of residence (2-3 days of field work)
• Schedule: Monday-Friday 8:00am-5:00pm

RN Care Manager Compensation

• The pay for this position is $40-$45 per hour based on years of experience
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN Care Manager Qualifications :

• Must live in McAllen, Corpus Christi, or Hidalgo TX area
• Must be bilingual in Spanish
• Active RN license in the state of TX
• 2+ years of Case Management experience
• Home Health or Hospice experience preferred

RN Care Manager Responsibilities :

· Provide support to STAR KIDS pediatric Medicaid members

· Conduct screening and risk assessment interventions per program guidelines.

· Identify special needs members through the completion of health screens and other resources.

· Work with community outreach/member advocates to coordinate member care.

· Educate providers and community resources on program components and available support services. Educate members with special needs to foster compliance with program and positively impact outcomes.

· Assist with modification and management of care plans, medical consenter or caregiver, and family, if applicable, as well as the managing physician

· Efficiency, quality and compliance with timeliness of required assessments and documentation. Process improvement to improve overall performance concerns; Seeing improvement on UMR cases/audits, improve member outcomes.

RN Care Manager Preferred Qualifications :

· Behavioral health background preferred

· Proficient with Microsoft programs, Outlook, Excel

· Familiar with Texas managed care programs/Medicaid and Medicare knowledge

• Integrate nursing case management with social work case management
• Identifying patient/family care needs
• Handle case intake and daily case management
• Provide telephonic medical case management strategy
• Serving in a case/care manager role
• Designated as the case manager for hospice patients
• Apply effective case management interventions
• Perform initial case management assessment to determine care coordination and discharge planning needs
• Evaluate outcomes of patient care
• Assuming nursing case management responsibilities for designated patients
• Identify members for case management
• Provide case management services to geriatrics patients
• Assume responsibility to coordinate patient care for assigned case load
• Manage the patient case from pre-admission through discharge
• Facilitating appropriate health care services throughout the continuum of care
• Identifying appropriate patients for care management
• Coordinate the overall interdisciplinary care plans for home health/hospice patients from admission to discharge
• Evaluate outcomes of care with the interdisciplinary team and medical case managers
• Providing patient case management services to assigned populations
• Performing ongoing telephonic case management and treatment planning
