Job Overview:
The Bilingual Case Manager supports Connecting NJ initiatives by coordinating referrals from the Connecting NJ HUB and linking families to appropriate community and early childhood resources, with a focus on those requiring a Plan of Safe Care. This role provides comprehensive care coordination, advocacy, and follow-up to ensure services are safe, effective, and client-centered. The Case Manager also identifies gaps and barriers within systems of care to improve access and equity for families.

Program Info
Connecting NJ (CNJ) isa network of partners and agencies in New Jersey dedicated to helping families thrive, especially those with new and growing families.It acts as a single point-of-entry system for various resources across state government, streamlining referrals and support for prenatal care, community agencies, and family well-being. In addition, CNJ works collaboratively with the Early Childhood Specialists (ECS). The ECS maintain strong relationships with local Division of Child Protection and Permanency (DCP&P) offices, assisting with DCP&P referrals and participate in case conferencing regarding Plans of Safe Care cases. The ECS focus on providing developmental resources, specifically the ASQ and ASQ-SE, to families.
Responsibilities:
- Participate in Connecting NJ initiatives and workflows to support overall program reach
- Case Manager will receive referrals from the Connecting NJ HUB for families who require additional resources and follow up.
- Will work closely with Early Childhood Systems of Care and existing resources to help link families to the most appropriate and available services, with special emphasis given to families requiring a Plan of Safe Care.
- Provide care coordination for families, research most appropriate resources and provide advocacy and follow-up as needed.
- Responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client centered
- Assists with identifying gaps and barriers to receiving services, and alleviate system level concerns and trends families experience with service utilization.
- Experience in maternal-child health (MCH), infant/early childhood mental health, parent/family support, and/or related fields and settings.
- Bachelor's degree required, preferred in Psychology, Social Work, Mental Health Counseling, Early Childhood Education or related education
- Bilingual - Spanish fluency
- Temporary coverage of other counties may be required when counterpart is unable to attend meetings or contact clients
- Base rate: $22/hourly
- Sign-on Bonus: $500
- Schedule: Monday-Friday 9a-5p; Hybrid position
- Bilingual Rate: An additional $1.50 added to the base rate if Spanish proficient.
