  Pay Rate: $28–$30/hr (W2)

Position Overview: The BHCM III will provide professional behavioral health case management and social work services in a psychiatric outpatient setting under the DSAMH CBHSD Bridge Program. The role focuses on supporting individuals with developmental disabilities, mental illness, or emotional disturbance to achieve social, emotional, and functional stability and independence.

• Provide professional behavioral health case management services

• Perform client assessments and treatment planning

• Deliver direct service provision using person-centered approaches

• Monitor client progress, risk, and safety

• Develop and manage service plans and case documentation

• Coordinate care and services with treatment teams

• Provide consultative support to state and private agencies

• Refer clients to appropriate social service resources

• Support comprehensive patient care aligned with DHSS/DSAMH standards

• Use counseling and interviewing techniques in case management delivery

Minimum Qualifications (at least one response required):

• Possession of a master’s Degree in Behavioral or Social Science or related field.

• Three years’ experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual’s human service needs using effective counseling or interviewing techniques as well as other available resources such as service plans or case management systems.

• Three years’ experience in providing clinical treatment, counseling, or determining eligibility for services or benefits as part of a treatment team.
