Location: Cape Girardeau, MO - Cape Girardeau County
Full Time: Exempt
Work Hours: Monday-Friday: 8:00 am - 4:30 pm. Some evenings may be required as necessary dependent upon agency and client need.

Pay Range: $21.69 - 24.09/hour - dependent upon prior experience



Minimum Required Qualifications:

  • Must meet MO DBH definition for Qualified Mental Health Professional (QMH))
  • Bachelor's Degree or higher in human services field (non-licensed) and/or is certified/credentialed to provide services within their scope of practice.
  • Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance. Must meet CCC's Motor Vehicle Record standards and have reliable transportation.


The Program
Community Psychiatric Rehabilitation Center (CPRC) supports adults and children with disabilities in social role functioning and daily living skills. Staff work closely with clients in various community settings, providing case management services and therapy to promote recovery.


The Job
Assessor is responsible for completing the CPRC initial and annual assessments as assigned.

Essential Functions

Complete initial, annual, and brief evaluations for clients enrolled to CPRC services as assigned.

2. Complete assessments in client's home or other location when needed to accommodate the client.

3. Complete assessments within the timeframe designated by CCC.

4. Use clinically appropriate language in assessments and follow agency guidelines regarding content of assessments.

5. Develop/revise individualized treatment plans for each client following agency guidelines.

6. Add required signers to the assessment/treatment plan and let signers know the document is ready for their

signature.

7. Ensure coordination of care by communicating concerns regarding client's mental health status or treatment with

CSC and CPRC supervisor.

8. Assist in screening clients for substance use disorders and provide diagnosis if clinically appropriate as needed.

9. Assess for client safety concerns and develop safety plans according to CCC standards/expectations.

10. Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per
