- Current APRN or PA-C license in the state of practice
- Minimum 3 years post-graduate experience in Primary Care, Family Medicine, or Internal Medicine
- (Urgent Care–only experience is not sufficient)
- Prior experience in value-based care (HEDIS, RAF/HCC coding) strongly preferred
- Experience with Athena EMR highly preferred
- Comfortable in a high-volume environment (23+ patients/day)
Clinical Excellence
- Provide comprehensive primary care for acute and chronic conditions
- Efficiently manage 23–24 patients per day using 20-minute visit templates
- Actively address care gaps (HEDIS) and ensure accurate HCC/RAF documentation at every visit
- Ensure patients receive all appropriate preventive screenings and follow-up care
Operational Efficiency
- Team Leadership: Direct Medical Assistants and Scribes to prep charts, scrub for care gaps, and pend orders prior to visits
- Chart Hygiene: Maintain a clean desktop with 95% of charts signed and closed within 48 hours
- Schedule Adherence: Follow visit templates to maintain steady patient flow throughout the workweek
The Advanced Practice Provider (APRN or PA-C) is responsible for delivering high-quality primary care to patients with both acute and chronic conditions in a fast-paced clinical environment. The role focuses on providing efficient patient care, addressing preventive care gaps, and maintaining accurate documentation while supporting value-based care initiatives. The provider will work closely with Medical Assistants and clinical staff to ensure smooth patient flow, timely chart completion, and high standards of clinical performance.
Key Responsibilities
- Provide comprehensive primary care for patients with acute and chronic conditions.
- Manage an average of 23–24 patients per day using structured 20-minute appointment templates.
- Identify and address care gaps (HEDIS) and ensure accurate HCC/RAF coding during each patient visit.
- Ensure patients receive appropriate preventive screenings and follow-up care.
- Lead and coordinate with Medical Assistants and Scribes to prepare patient charts, review care gaps, and pend orders before visits.
- Maintain accurate and timely EMR documentation, ensuring 95% of charts are signed and closed within 48 hours.
- Follow scheduling templates to maintain consistent patient flow and clinic efficiency.
- Deliver patient-centered care while supporting value-based care initiatives and quality metrics.
