Job Details

Full‑time, field‑based Hospice Admissions RN role (3-4 patients a day)
Perform initial physical, psychosocial, and health system assessments to develop individualized plans of care
Conduct admissions in patient homes, hospitals, and nursing facilities in compliance with hospice regulations
Collaborate with interdisciplinary teams and engage in community outreach with physicians and providers

Requirements
Graduate of an accredited School of Nursing (BSN preferred)
Active New York State RN license; CPR‑BLS preferred
Minimum of two years of nursing experience (Medical‑Surgical or Community Health; hospice or oncology preferred)
