EXCENTIA JOB DESCRIPTION

Job Title:    Program Supervisor (CNA)

Department:    Residential Services

Reports To:    Associate Director

FLSA Status:    Exempt

Revision Date:  July 27, 2016, November 16, 2016, December 1, 2017, October 15, 2017

Hours:  Full-time; flexible to meet the needs of the organization and position; available for evening and weekend hours as needed.

SUMMARY

Monitors and ensures that the needs of all the individuals residing in the home are being met. Supervises and coordinates all personnel assigned to the home by completing the following duties.

ESSENTIAL JOB FUNCTIONS/MAJOR TASKS

a.  Participates in the referral process including initial visitations and orientating the individual and family/guardian to the home. Complete the initial and on-going required paperwork including admission forms, assessment forms and the establishment and maintenance of individual records for all residing in the home.

b.  Responsible for ensuring that individual program planning takes place, including the completion of daily, monthly and quarterly progress review forms. Provide input into the development of and responsibility for the implementation of steps towards meeting established individual outcomes as stated in the Individual Support Plan (ISP). This includes training staff in the implementation of individual support plans.

c.  Monitors individuals' health needs, including scheduling required appointments, accompany individuals to appointments, administer necessary medication, and ensure that all required paperwork is accurately completed. Ensures that there is training for each direct service worker in the content of health and safety needs relevant to each individual residing in the home.

d.  Acts as a role model to staff in providing direct care to the individuals periodically and as the situation dictates, always keeping the best interests and needs of the individuals in mind.

e.  Professionally and rationally addresses immediate individual problems in collaboration with program staff, reporting serious problems to the Associate Director immediately.

f.  Assists in the on-site interviewing of potential staff and making recommendations for employment in the home.

g.  Supervises all personnel assigned to the home, including monitoring of performance, training of new staff, ensuring that staff complete all required training, completion of annual performance evaluations, and monitoring staff performance on a daily basis.

h.  Periodically completes unannounced visits to the home to monitor staff performance and ensure regulatory compliance.

i.  Coordinates regular in-house staff meetings to facilitate communication and consistency of goals/outcomes, optimum level of care and minimize chance of risk to employees and the individuals in our care.

j.  Coordinates staff schedules, including contacting substitutes, scheduling staff for all time periods when the individuals are present in the home, organize staff schedules to ensure efficient use of staff time and eliminating overtime wherever possible. Ultimate responsibility for coverage due to staff shortages if no other substitute is available.

k.  Keep all assigned staff aware of agency happenings. Ensure that all agency communications, including memos, job listings, policy changes, etc. are forwarded to staff in a timely fashion.

l.  Possesses a thorough working knowledge and ensures compliance with agency policy and procedures, state regulations, county guidelines and any other relevant regulations.

m.  Assures accurate completion of all required reports and forms including medical reports, house budget, individual expenditures, facility/equipment repair requests, reportable incidents, programmatic reports, fire drills, payroll submissions, inventory, etc.

n.  Assists with internal licensing inspections and preparation for annual licensing.

o.  Acts as a good neighbor in the workplace setting by ensuring the upkeep of the property, demonstrating expected neighborly etiquette and sharing these qualities with the employees working in the home and with the individuals living in the home.

p.  Serve as a welcome host for any visitors to the home.

q.  Coordinate the completion of and participate in the maintenance of the home to ensure high standards in regards to its’ cleanliness, home-like environment, safety and overall appearance.

r.  Knowledge of emergency procedures and reporting requirements, including web based reporting requirements. Serve on the emergency on-call team as a point person and serve as a Certified Medication Trainer or Practicum Observer at the discretion of the Director.

s.  Works independently, but conveys observations and key issues to the Associate Director or in their absence, the Director on a regular basis.

t.  Attends and conduct meetings and training as required. Completes a minimum of 24 hours of training relevant to human services training each training year.

u.  Works in all circumstances in a manner consistent with the Core Values of the organization.

v.  Any other duties and/or special projects that may be assigned.

SUPERVISORY RESPONSIBILITIES

Responsible for the overall direction, coordination, and evaluation of the home. Directly supervise all staff assigned to the home. Carry out Supervisory responsibilities in accordance with all established policies and procedures of the agency as well as all applicable regulations. Responsibilities may include interviewing, staff training; planning, assigning, and directing work; appraising staff performance; rewarding and disciplining employees; addressing complaints and resolving problems.

The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move more than 100 pounds with assistive devices, i.e. lift, cart, etc. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts, fumes or airborne particles, and outside weather conditions. The noise level in the work environment is usually moderate, but can be loud at times.

1.  COMMUNICATION

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write detailed reports and professional correspondence. Ability to speak clearly and express self well with a diverse group of individuals, family members, staff, vendors, professionals, and medical personnel. Computer skills, ability to navigate the internet and complete web-based reports is required.



2.  MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

3.  REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to think independently, gather relevant information systematically, grasp complexities and use problem solving skills on a daily basis. Ability to act with clarity and in a timely manner.

4.  EMPLOYMENT EXPERIENCE

Minimum of 1 year (preferably 2 years) of experience working directly with persons with intellectual/developmental disabilities is required and at least 1 year of demonstrated leadership experience is required for this position.

5.  OTHER

 Valid and current Pennsylvania Driver’s License.

6. EDUCATION

A current Certified Nursing Assistant Certification (In good standing). Must be maintained throughout employment as Program Supervisor.
