Nurse - Case Manager

Job Description

PRIMARY RELATIONSHIPS: Medical Director, Home Care Coordinator, Clinical Services Manager, Center Manager, Executive Director, Center staff, participants, family members and public.
OBJECTIVE: To provide intense, comprehensive medical case management of PACE participants by partnering with various entities (hospital, physicians, IDT) in order to manage care and treatments aligning with the participant's pathway and PACE philosophy, reducing the need for institutionalizations.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before have direct participant contact.
2. Must have a valid driver's license, proof of insurance and have means of transportation.
3. Education: Graduate of a school of an accredited school of nursing with Bachelors of Science Nursing (BSN). *Enrollment in a qualified BSN program may be given consideration with a combination of experience. Current State Registered Nurses License, and Cardio Pulmonary Resuscitation (CPR) certification required.
4. Experience: Has practiced nursing in the last five (5) years. A minimum of three (3) years of experience in working with the frail or elderly population required.
5. Must have experience working collaboratively with physicians and medical providers utilizing appropriate communication styles suited to the situation.

ESSENTIAL FUNCTIONS:
1. Follow participants with acute hospital admissions by daily visits, coordinating with hospital medical providers and collaborating with discharge planners for discharge service needs.
2. Follow participants post-acute and ER discharge, coordinating care with IDT (Interdisciplinary Team).
3. Follow participants in skilled/rehabilitation setting.
4. Follow participants in long term care when change of condition occurs.
5. Involved in coordination of care of participants with the following:
* Transplant consideration
* Oncology
* Complicated medical conditions
6. Attends medical appointments as needed for participants with:
* Dementia
* Complicated medical conditions
* Surgical procedures being considered
7. Directly involved in communicating with participants and families about advanced care directives, in particular participant change of status with medical decline, and End of Life consideration.
8. Reads, records and fills out the PMIP's for the county for potential participant enrollment (initial and re-certifications).
