Job Description

Become part of Heart to Heart Home Care’s extraordinary team of caring professionals.

As the Team Leader of Coordination, you will lead and supervise the coordination team.

You will assist with all aspects of Heart to Heart Home Care’s operations and be part of a compassionate and professional team helping families and their loved ones in their time of need. Successful candidates must be extremely detail-oriented, self-motivated, with excellent communication, administrative, clerical and problem solving skills.

Responsibilities include:

  • Assist in managing daily office activities
  • Match, assign and schedule caregivers based on skill level, availability and customer requirements
  • Provide superior customer service and compassion for new clients and their families
  • Receive and filter incoming calls
  • Maintain and manage staff and customer files
  • Receive and process incoming requests for new home care services
  • Ensure compliance with all regulatory, as well as, internal policies and procedures
  • Participate in after-hours, weekend and holiday coverage program

Specific requirements include:

  • Must have a Great Welcoming attitude
  • Superior customer service and strong communication skills (written and verbal)
  • Excellent administrative and clerical skills
  • Highly detail oriented
  • Proficiency with computers (Microsoft Word / Excel), and ability to learn and operate scheduling and training software systems(Sandata, HCPLUS, HHA Exchange or Santrax)
  • Ability to prioritize and handle multiple tasks
  • Remain composed, professional with a positive attitude under pressure and excel in time management
  • Must have a computer, laptop and reliable internet access from home

Applicants must provide references who can confirm relevant prior work experience. (References from any Managed Long Term Contract a PLUS**)
