We are covering 100% for the medical, dental and vision insurances. Responsibilities: The Home Health Registered Nurse (RN) will conduct a detailed assessment of the patient & family during the initial start of care. Develop a plan of care with the collected start of care data. Case manages patient care episodes and works one on one with patient/ family to improve health and stability. Complete clinical notes, orders, and charting in a timely manner. Report and consult with physicians and care team regarding patient care and goals. Qualifications for the Registered Nurse (RN) include but are not limited to: Current California RN License Current CPR/ BLS Certification Valid Driver's License 1 Year: RN/ Home Health experience required. 3 Years: RN/ Home Health experience preferred.
