Program Manager - Mobile Medication Program

Glade Run Lutheran Services Butler, PA (Onsite) Full-Time

Glade Run has launched a Psychiatric Mobile Medication program to serve Butler County adults 18 and over with a diagnosed serious mental illness, a history of repeated acute inpatient admissions, and patterns that include poor medication adherence and/or consistent substance utilization post discharge.

The goal of the program is to prevent hospitalization caused by limited insight into illness, lack of understanding of the purpose and benefit of medications, and other factors. The program will provide needed assessment, support and skill development to those individuals who have a high degree of functional impairment associated with medication adherence that impacts their illness management and recovery.

POSITION SPECIFIC CORE COMPETENCIES:

  • Responsible for providing oversight and monitoring of client medications; education regarding medications; linkage with appropriate resources and supportive case management services.

  • Initiates policies and procedures necessary for the operation of the Mobile Medication Program in consultation with the Vice President of Quality and Compliance.

  • Participates in the agency hiring process and supervision of the Mobile Medication Program staff.

  • Supervises mobile medication staff in the completion of direct service activities to include but not limited to medication monitoring via individual RN med checks or co-facilitation of medication groups with prescribers; nursing assessments and coordination of care; provides medication education related to side effects, efficacy and expected outcomes.

  • Ensures financial integrity of the program via coordination of managed care authorizations, schedule of service delivery and staff coverage.

  • Collaboration with stakeholders and other providers for linkage to appropriate resources and supports.

  • Participate in Quality Improvement activities to enhance positive consumer outcomes.

  • Arranges staff coverage during vacations, absences, etc. in the Mobile Medication Program.

  • Provides a minimum weekly individual and/or group supervision to Mobile Medication Workers.

  • Participates with the Vice President of Quality and Compliance in the evaluation of the program operation.

  • Responsible for administering prescribed medications, performing physical assessments and other nursing skills as required to the Mobile Medication Program.

  • Ensure medications are stored and administered in accordance with all applicable regulatory entities.

  • Order and ensure that all medications on site have proper medication orders present and are accurate.

  • Performs medication counts: Daily for controlled substances, weekly for all other medications. Reports discrepancies immediately to supervisor, completes all corresponding documentation

  • Provides oversight in the referral process; development of treatment plans, treatment reviews and other related documentation of the mobile medication program.

  • Attends staff meetings, conferences and in-service training as required.

  • Performs other related duties as may be required or requested.

Qualifications

QUALIFICATIONS:

  • Bachelors degree in social services OR Licensed Registered Nurse, Physician Assistant or Licensed Certified Registered Nurse Practitioner.

  • At least two years of experience with seriously mentally ill patients.

  • At least one-year supervision experience in the mental health or physical health profession.

  • Working knowledge of psychotropic medication.

  • Good verbal and written communications skills.

  • Ability to make sound judgements regarding client care.

  • Must have a valid drivers license, acceptable driving record and be willing to travel throughout Butler County.

  • Must have acceptable PA State Police clearance, PA Child Abuse History Clearance, FBI Clearance, and Arrest/Conviction Report and Certification Form.

  • Must be able to identify with the agency Mission Statement and support the agencys Core Values.

  • Must complete and pass agency Crisis/Physical Intervention training.
