Position Description
The Registered Nurse provides intermittent skilled nursing services and directs, supervises and instructs hospice aide staff in the provision of personal patient care. As a Registered Nurse you will:
Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care
Report significant findings to patient’s physician and Clinical Supervisor as they occur
Submit completed skilled nursing visit notes and documents, and discharge planning process per Medicare criteria as requested by Clinical Supervisor
Attend staff meetings, team conferences, and educational in-services as needed
Qualifications
A current unencumbered State Professional Nurse License
Must maintain a valid driver's license and good driving record
Ability to work in a field setting and exhibited ability to make sound nursing judgment
Ability to assess patient needs and formulate individualized patient care plans to meet those needs
Experience with Microsoft Office
