Description

The Refugee Health Program (RHP) Nurse will provide clinical support to providers during the initial health assessment and follow-up visits of newly arrived Humanitarian Immigrants, following the requirements set forth by the Maryland Department of Health (MDH) Office of Immigrant Health. The RHP Nurse is a Registered Nurse responsible for coordinating the clinical activities of the Refugee Health Program including but not limited to overseeing the delivery of clinical care during Refugee Health Assessment (RHA), pre-vit planning, mental health screenings, as well as implementing care plans and conducting health education for Humanitarian immigrants. The RHP Nurse Will serve as lead for the Refugee TB program at BMS and serves under the direction of the Director of International Services.

Essential Functions:
  • Manages the flow of the clinical team's daily schedule.
  • Conducts Refugee Health Program visits, including review of international medical records and identification of urgent health needs before the patient's visit.
  • Performs behavioral health screening on all patients at least 14 years of age with Refugee Heath Screener-13 (RHS-13) tool. Refers patients to behavioral health according to RHS-13 results or as deemed appropriate by RN.
  • Participates in daily huddles in a manner consistent with PCMH initiatives.
  • Provides and oversees clinical coordination with the patient and the referring agencies in collaboration with the Refuge Care Coordinator.
  • Attends grant-related meetings and training.
  • Conducts patient assessments and screenings, following established standards and requirements.
  • Serves as lead for Tuberculosis care.
  • Counsels patients on the use and side effects of prescribed medications and assists with medication reconciliation.
Requirements
  • Graduate of an accredited school of professional nursing by hire date. Preferred BSN
  • Preferred: BSN; 2 years of experience working in HIV prevention or care, medical care systems, diverse populations, and/or underserved communities
  • Registered Nurse in the State of Maryland (or compact state)
  • Unexpired CPR certification from the American Heart Association or American Red Cross
  • Spanish speakers preferred
