Job Description
We are looking for an RN to add to our Home Health agency. The RN is to provide care and support to geriatric & adults patients. A Home health nurse provides services to patients in the comfort of their homes, offering a welcoming alternative to frequent and prolonged hospital stays. They offer a full continuum of care - everything from wound care and medication management to home infusion services and monitoring the general health of the patient. For this role, you need to be a Registered Nurse who sees the value of home health post-acute care services which involves assisting patients with basic needs such as bathing and dressing, as well as more specialized services such as wound care, medication management, and IV therapy. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position.

Responsibilities:

• Provide care to patients based on their care plan outlined by their doctor, in their home
• Accurately monitor and record observations on patients' conditions, to include: exacerbation's of a patients disease process, assess & record vital signs, report any abnormalities to Director of Nurse
• Maintain accurate records of patients care, medication administration and monitor patients' diet and exercise
• Identify care planning needs
• Carry out risk assessment
• Evaluate patient care plan as conditions change
• Consult and coordinate with other healthcare team members
• Make frequent visits to the patients home to evaluate their condition
• Provide support and comfort according to individual patient’s needs
• Work with caregivers to plan and execute excellent patient care
• Notify a doctor for a patient’s condition when deemed necessary
• Assist patients and their families with paperwork
• Maintain accurate records
• Participate in on-call rotation (paid in addition to salary)
