Qualifications
- High school diploma or equivalent
- Valid Michigan RN license
- Minimum of one year of current, continuous experience in an acute care setting is preferred
- Must have a dependable vehicle with current driver's license and auto insurance coverage
- BLS certification (available through Hillsdale Hospital)
- Proficient with computer software used in healthcare
- Calls patients to schedule visits
- Evaluates patients' and residents' health and implements plan of care in collaboration with the patient, family, and physician
- Administers treatment, medications and IV solutions according to physician’s orders
- Observes and evaluates symptoms and reactions to treatment; initiates corrective action if needed, updating plan of care, and advising physician of changes in patient's condition
- Documents and maintains records and reports regarding nursing care provided
- Must complete OASIS information and perform the comprehensive assessment as SOC, resumption, transfer, death in the home and at discharge
- Coordinates services to facilitate holistic patient care
- Maintains daily visit productivity of 5 to 7 visits adjusted for travel and other assignments
