Northern Light Mercy HospitalDepartment: Care ManagementPosition is located: Cianchette Corporate Office, Brewer, MEWork Type: Full TimeFTE: 40 Hours WeeklyWork Schedule: 8:00 AM to 4:30 PMSummary:The VP Integrated Care Management (ICM) will be responsible for the execution of the strategic vision across all areas of Northern Light Health for Ambulatory Care Management and Navigation, including planning, directing, and coordinating system initiatives. The principal focus is to achieve high quality, cost-effective care across the continuum. The VP ICM requires a balance of evidence-informed clinical knowledge, strong business acumen and effective collaboration at the enterprise, hospital, ambulatory practice and community levels with multi-disciplinary clinicians, senior administrators, and executives. The VP ICM will be a highly engaged, visible leader adept at articulating and guiding execution of high quality, patient-centered care across a range of diverse settings within the system. The VP ICM provides oversight for Care Management and Navigation services across the system and is part of a matrix relationship with the SVP & President Beacon Health and provides direct supervision to the AVP of Care Management, Navigation, Ambulatory Service Lines.Responsibilities:
• Provides strategic and operational oversight for the Ambulatory Care Management, Community Care team, and Cerner Patient Observer programs
• Serves in a matrixed role with strategic partners for ICM for Acute Care Management and Utilization Review
• Translates, executes, delivers, and drives established initiatives set forth by the Clinical, Financial and Operation leaders
• Acts as a leader and/or liaison to identify and resolve Care Continuum Management (CCM) issues impacting service delivery and escalates to appropriate stakeholders where appropriate
• Collaborates with the Clinical Executive to develop policies and standard operating procedures (SOP) and identifies best-practices for Care Continuum Management
• Collaborates with the ICM/UR strategic partner to standardize and implement best practices for ICM/UR
• Responsible for executing and enforcing policies, SOP, and best practices
• Promotes system thinking and integration as an expectation of leaders and staff across the system
• Monitors and coordinates processes and operations around research/seed programs
• Builds strong relationships with external and internal customers
• Prioritizes and implements ongoing performance of new and existing initiatives and develops recommendations/identifies opportunities for performance improvementOther Information:BLS Required.
•Master’s Degree in Nursing Administration, Business Administration, Health Administration, or related field required
•Active Maine RN license or eligibility to obtain preferred
•At least seven (7) years of progressive experience in leadership roles including experience at an executive level (AVP or higher) is required
