Patient Care Coordinator
Busy homecare agency located in central New Jersey seeking a Patient Care Coordinator to coordinate and manage patient cases.
Candidates for the Patient Care Coordinator position must be quick learners, able to multi-task, have excellent written and verbal communication skills, and the ability to work independently and as a part of a team. They must want to learn, grow, and evolve with the company by achieving company objectives and goals.
Daily duties for a Patient Care Coordinator can include:
- Communicate with patients and caregivers to ensure cases are staffed.
- Update case managers as needed on patient status.
- Liasing between other departments as needed.
- Staffing patient cases completely and to patient satisfaction.
- Managing the caregivers assigned to cases.
- Ensuring that caregivers remain EVV compliant.
Experience needed for a Patient Care Coordinator:
- Previous job experience OR college education
- Experience in homecare or patient care
Relevant Skills for a Patient Care Coordinator:
- Strong team player
- Self-motivated
- Excellent written and verbal communications
- Strong phone skills
- Computer skills
- Bi-lingual (Spanish)
